BDAC was founded to represent and provide services to Aboriginal and Torres Strait Islander people living on Djaara Country.

About the Position

The Chronic Disease Management (CDM) team at BDAC enhance patient care and reduce Emergency Department GP like presentations (including non-emergency dental presentations) by implementing an intensive care coordination model. This model will identify and support high-risk patients, streamline comprehensive health assessments (715 assessments), improve access and engagement, and foster collaboration with healthcare partners while ensuring cultural safety. 

The Chronic Disease Management Registered Nurse will play a pivotal role in providing high-quality, culturally sensitive care within our Aboriginal Community Controlled Organisation (ACCO). This position involves working alongside a dedicated team of nurses in delivering comprehensive, patient-centred chronic disease management services. The Registered Nurse will ensure that care plans are tailored to meet the unique needs of our community members, participate in clinical practices, and collaborate with other healthcare professionals and community stakeholders to improve health outcomes.

Key Responsibilities

  • High-Risk Patient Management: Support high-risk patients through tailored care plans and regular monitoring, focusing on improving health outcomes and preventing complications/hospitalisation.
  • Case Management: Oversee the case management of patients with complex needs, ensuring they receive comprehensive and continuous care by tracking patient progress, managing care transitions, and addressing any barriers to effective treatment.
  • Comprehensive Health Assessments: Streamline the completion of comprehensive health assessments (715 assessments), ensuring thoroughness and accuracy in evaluating patient needs.
  • Collaboration and Networking: Build and maintain strong relationships with healthcare partners, community organisations, and other stakeholders to facilitate seamless access to services and enhance patient support.
  • Reporting and Evaluation: Compile, analyse, and report on clinical data, patient outcomes, and operational metrics to the Manager of the Chronic Disease Team, ensuring timely and accurate information to support strategic decision-making and continuous improvement.
  • Community and Stakeholder Engagement: Build and maintain relationships with community, within BDAC, and with local organisations, healthcare providers and other entities to ensure effective collaboration, and the best outcomes for the community.
  • Client Advocacy: The CDM Program Registered Nurse will advocate for community with the aim of culturally appropriate care and support.


Skills And Experiences

About You

  • Demonstrated knowledge and understanding of Aboriginal Culture and the ACCO environment.
  • Demonstrated experience in providing acute nursing care.
  • Proven Adaptability and Innovation: Willingness and ability to adapt to changing healthcare environments and adopt innovative strategies.
  • Experience in stakeholder engagement and management across a range of healthcare sectors.
  • Demonstrated experience working with patients with chronic disease and comorbidities.

Preferred / Desired / Mandatory Education, Training and/or Competencies.

  • Bachelor of Nursing – Registered Nurse with the Australian Health Practitioner Regulation Agency
